A bug fix was built on a local copy of a file, then ready to go live. Between the time the local copy was made and the time the fix was completed, the live file had been separately updated in a background session with unrelated changes (a hero redesign, new related-links). Applying the fix from the local copy would have silently dropped both of those additions.
⚡ Quick Fix (TL;DR)
The Culprit: Assuming a local copy remains authoritative after time has passed. The live file was never consulted to check for interim updates.
The Fix: The actual current file was uploaded and diffed against the fix before accepting it as final. This surfaced both unrelated changes that would have been lost. Discipline: always diff against the live file, not a local copy that might be hours old, no matter how recent it felt when it was made.
